My own guess is that the next great public health crisis may well be an old one: TB. The bug develops resistance at a prodigious rate, largely because it requires such an extended course of treatment, and communicates extremely easily. It's everywhere over here, and in most of the developing world; I suspect that there's more of it there than people want to talk about.

if there is a 'breakout' the powers that be will reevaluate the inoculation policy

if there is an epidemic, vaccines will be made available quickly

I would guess it would break out in a 3rd world country, but virus's can become global quickly in today's world
