Bad Cable Can Be Solved

Yes and no. It can be solved by an MSO that is willing to make the investment in new plant. It will not necessarily be solved by an MSO that is unwilling or financially unable to make the necessary investment, & is only interested in shortcuts & minimal upgrades.

DSL Outside 3 Miles Will Never Be Solved

Good advice is to never say never. More on point is to define what you mean by the "3 mile zone"? Do you mean loop distance from the CO? Or is it the more accurate definition of the copper loop distance the DSLAM is located from the subscriber's premises.

You might want to do some reading on what SBC is doing along the lines of their Project Pronto. Essentially, they are laying fiber to remote terminal (RT) sites close to neighborhoods so that no subscriber, or no less than 80% of their current telephonic customers, are within 12K ft of that RT. That RT will contain in a closed environmental vault (CEV) next general digital loop carrier (NGDLC) equipment that incorporate a DSLAM as part of the set-up. This is how SBC plans to overcome the "distance" problem in most of its service areas. IMHO, other incumbent carriers will follow a similar strategy.

OTOH, technology does exist to extend DSL beyond the 3 mile limit, but at the cost of more narrow bandwidth delivered to the subscriber. Larger gauge cable is more tolerant of distant constraints, so if the telco's really wanted to move out to 4 to 4 1/2 miles they could, but that's not where the focus is. G.lite has greater distance tolerances than DMT, but the focus today is on expanding the bandwidth, not the copper loop distance. Focus is on laying fiber deeper into current copper plant, moving distance critical equipment closer to the subscriber, and delivering a multitude of advanced services, that IMHO, will blow your socks off as a residential customer in a few years.
