MSO = multi-system operator, aka as a cable company. You local cable company operates a localized cable network system pursuant a franchise granted by the local franchising authority, typically a local city or county government. TimeWarner, for example, operates numerous franchises throughout the US. Often several clustered franchises are operated under a single cable system, or network. Very often these operators own numerous systems throughout the country. Hence the name multi-system operator.

The name MSO most likely finds it root in cable legislation, which defines the types of operators subject to the Cable Act. The Act likely requires the operator to be engaged in some level of interstate commerce to fall in order to fall w/i the Act's jurisdiction. Hope this helps.
