mental clearance were required before a permit were issued This type of event is why I propose that gun owners need to go through a process every 5 years or so that shows that they still are in control of their weapon. Besides determining if it was lost, stolen, sold, or given away, there would be an opportunity to re-check the background. It probably wouldn't catch all the crazies or the criminals, but in many cases it would expose their potential (Often because they would refuse to comply) before a tradegy instead of cleaning up after.
