I think you did a good job, Cobalt. But if I were one of the doctors, I think I'd just like to see a timeline, starting with the Sunday night when she (later) reported having become ill. Day by day, in chronological order, with what she said about her symptoms when. That should be the first section.

Then you need a second part, outlining what she'd said in the past about chronic health problems, and the types of drugs and herbal remedies that she regularly or occasionally took.

And then a third part detailing her recent activities: any trips, unusual contacts, visits to restaurants, the business about the weekend guest(s), the housecleaning project, the leakage from the upstairs apartment, and so on.

Keep it as brief and factual as possible.

I think any doctor who wouldn't welcome a short summary which might be helpful wouldn't be a very competent doctor. I would definitely send it. When one is clutching at straws, any straw could hold the answer.
