My guess is that your server doesn't have the FrontPage Server Extensions installed - from your error message, it seems that its not finding the WEBBOT, which is a FrontPage thing, not a Web or Apache (the Web server that you are using) thing.

With regard to your html problem,

I believe the problem is with the path of the U-File. You say your directory is:

zoominternet.net

Notice the ~ preceding your name.

The path of the U-File(Where the info from the form is saved) is:

zoominternet.net

Notice no ~ before your name. I believe this is the problem.

If the results page is located in the same directory as the form page, you can use relative addressing, that is
U-File="results.html"
