Was the 1994 bankruptcy disclosed in any SEC filings?

I can't find any mention of it in the 1998-1999 prospectus filed on form SB-2 and SB-2/a.

The first form 10k that was filed was October 14 1999, which was 5 years plus 4 months after bankruptcy was filed. Apparently after 5 years, the company doesn't need to disclose personal bankruptcies. Here's what the 10k said:

Except as indicated below and or hereinbefore, to the knowledge of
Management, during the past five years, no present director, executive
officer, or person nominated to become a director or executive officer of the
Company:

(1) Filed a petition under federal bankruptcy laws or any state
insolvency law

I can't find any such statement in the SB-2, which was filed within the 5-year period. Nothing appears in that prospectus at all, on the issue of personal bankruptcy. edgar-online.com
