I too use that as a rule of thumb; however Im not that firm to say that it must start that way. Im a firm believer that things change especially in such a dynamic market, so Im not one to hold onto firm rules all the time.

i just draw trendlines from "eye". i dont really follow patterns to the "t". TA -- there are no hard rules.
