the CBOE trades index and equity options.
so the EXCHANGE P/C is for all options traded there, but most ppl only track the CBOE EQUITY P/C.

btw, this is CBOE EQUITY P/C VOLUME

there are all other kinds of P/C ratios... including P/C open interest and variations of, including all the option exchanges or not, volume or OI, etc.

I'm not sure of the historical significance of tracking only *CBOE* P/C, other than that the CBOE was the first for most things options related, and i believe is still the biggest (largest volume).
