When will the actual players be attacked from the outside and users stop being squeezed into the below 1Mbit/s rates?

ADSL CLECs tried via unbundling. It won't work because they will be hunted and killed by RBOC's (see my comments on Tom Nolle's article. See also your bear-hug theory).

Tom Nolle forgot the basic: Solutions integrated into the switch are not preferred. I don't belive in those 'wrap around' solutions ERiCY Engine, SMAWY Surpass, NT's Succession or LU's 7RE.

Operators will be at mercy of the LU, ERICY, NEC, ALA and SMAWY of this world. Software upgrades are implemented at a glacial pace.
New services must be implemented off switch to have a chance.

Technically there are three possibilities:
CATV via cable modems
LMDS
Wireless data (3G UMTS and such)
FTTH

The attack will surely come via the Last Mile
