transmission has discussed MMDS equipment providers
in detail on several occasions (your list should
include CAMP). On the LMDS equipment side, you might
want to keep an eye on Ensemble Communications which
is private but has an impressive list of sponsors
(CSCO, ADCT, STXN, and I believe, INTC).

Another angle you might want to take is to keep an eye

Do you have any thoughts about fixed wireless over the mobile bands? (CDMA GSM or TDMA).

Seems like a great way to provide local phone service in areas not served by copper around the world and also data once 2G and 3G start being deployed commercially over existing infrastructure. I follow a company called Telular which produces FWT's for all three standards and they hold several patents concerning connecting standard telephone equipment to RJ11 jacks which generates a dial tone so the wireless connection is transparent to the user.

To date the market is very small but interest seems to be picking up as a last mile solution.
