Gw, it is ethically accepted, and VERY commonly use to do trial with minimal oral consent once institutions have agree to "general consent".

The most vivid example is the last year trial in Thailand, Caribbean and South Africa using "short term" azt prevention as opposed to the US accepted protocol to prevent Aids virus perinatal transmision.

It was considered that the population does not have acces anyway to prevention, so it is morally acceptable to use a "Placebo" group WITHOUT drugs and for sure afectting up to 15% of the probable avoidance of the virus with it, and KNOWINGLY.

Thailand goverment up to fund their part of the study and all the subjects received either US standard or "new short term".

This trial became a political problem for the nominated US Surgeon General, cause he (happens to be black)was the head at the Center for Disease Control that sponsored the trial and he personally give the go ahead to it.

Only oral consent was obatined and "surrogate" consent from care givers/clinicians was accepted.

"Surrogate" concern is use everyday in clinical medicine when the patient can not consent, no family is available and intervention goes on anyway (for proven therapies, of course. And this is considered a moral equivalent to the former research consent.

The end of the hiv trial prevention was highly succesful and it is now being use worldwide with the sponsorship of Glaxo/Wellcome providing the drug and logistics of distribution free, for the benefit of thousands and probably in few years millions of children.

Respect and honor for the affected ones during the trial. Victims of the real world.
