A close family friend (and Professor of Psychiatry) was part of a group that was invited to the Soviet Union during the early days of Perestroika. She visited some "mental institutions" there and was rather appalled by them. Although a few well meaning individuals were attempting some therapy, those institutions were mainly political prisons, with some provision for warehousing the severely disturbed. The notion of using such facilities for treatment of emotional disorders was almost bizarre to most of those who had been working in them. The most basic knowledge of psychiatric treatment was missing. Imagine some poor soul sent to one those places in desperate need of treatment (one of the more strident Rambus longs, for instance).
