I don't see how proliferation cancels out the efficiency of MAD. MAD works because it employs the most basic of all human instincts: self-preservation. Hard to do much better than that.

A world with more countries with nuclear weopons makes MAD
deterence less certain and mistakes more likely. It also subjects us to potential nuclear blackmail by minor powers in desperate situations or by the unlikely but still posible
rouge general or well placed and lucky terrorist orginization that manages to get a hold of a nuclear missle.
(When few countries had missles and they were stable this risk was just about zero, now it is still low, but it is something I think we have to be ready for).
