You have to tape out again, but not necessarily all layers. And if the new tapeout is just a minor bug fix, production can begin with previous masks.

If the chip is not totally brand new, the platform is sound and the process is well characterized, a 9 month window is aggressive possible. It takes a lot of things to line up properly to make that happen. In P4's case what Intel is doing is a testament to the amount of work being done to make P4 happen. I would consider a November launch to be a good effort.

Not really. Everytime a change is made to the design a new tapeout occurs resulting in a new stepping. The complexity of the change can be simple and take just a few days(typically you hear stuff like metal mask change) or complex (typically you hear all layer change or major revision and that could take weeks)
