If you know a Telco from the inside you would expect those problems. But people put their mouth in fifth gear and their brain in first gear without knowing how a Telco who is there to make things happen.
Cable Plant in a Telco is managed, operated and maintained by Outside Plant people -and it is people that make things happen- and this type of people is not the ones best prepared to handle such a roll out like ADSL.

Look the number of consultants they are hiring to solve the problems. If ADSL would be the real thing, long time ago the external plant should have been split from the telco.
