I understand that this was a redesigned ballot (by local officials) to accomodate large type. It was used in Palm Beach. I don't know if it was county-wide or certain precincts yet.

There were at least two problems: The change in format to a dual sided ballot (butterfly) which confused voters who were accustomed to a single sided (left) ballot. The confusing ordering and numbering of candidates with the punch holes.
