As far as we know, some people thought they could vote twice for president. Why didn't they simply ask for a new ballot? Who is ultimately responsible?

Let's stick to the facts, which are that 19,120 ballots were rejected because they voted for multiple candidates for President. No doubt some of those voted for both Gore and Buchanan, but to this point there is no indication of what that number is.

Actually, they just said 19,000 ballots were thrown out, because they had multiple punches, but it didn't say what (multiple) candidates had been punched.
