Since the Dems approved the ballot prior to its use, are you suggesting they are either ignorant of the law or willfully tried to circumvent it?

Yes, I agree that the Presidency should not hinge on a mistake. But, is it not the responsibility of the voter to get another ballot if they made a mistake? 96% of the voters turned in correct ballots. The other 4% made mistakes and must accept responsiblity. Unfortunately, that mistake may cost their candidate the Presidency. That is extremely unfortunate and it is the law.

Someone before me brought up a particular golf tournament where the leader signed an incorrect scorecard and lost the tournament because that's what the rules of the game state.

Clearly, this election is no golf match. However, the rules are the rules.
