No, it doesn't. Don't believe everything you hear on TV. There's a model ballot and the statute says that the real ballot shall conform substantially to the model ballot. Substantially means good enough to accomplish the purpose, but it's really a weasel word. But there's no provision for invalidating an election if the ballot didn't conform to the code. The ballot has to be challenged before the election. You can look at it for yourself, I've cut and pasted it to the thread.
