Multiple people should document such ballots and carefully store them along with the reason the ballot cannot be condsidered valid, and whether a replacement ballot was issued. It seems to me that not doing so makes tampering more possible. All it takes is one dishonest poll worker to decide to trash a bunch of ballots. I don't think any poll worker should ever be allowed to be alone with a ballot, whether it has been completed or not.
