You insert a punch card into a slot that guides it up underneath the "ballot" template that you've seen on TV. There's a little needle or pin that you poke through the hold (next to the arrow, ya know), and then the cards are fed through readers when they are counted.

Got this (or something like it) from CNN's chat thread:

Florida's new vote machine will work like this. The voter enters the booth and has the option of punching a real big hole that says, "I am incapable of accurately punching little holes that require that I read and follow instructions".

The vote machine will then automatically punch all democratic candidates contained on the ballot.
