That has always been the term for the little squares knocked out of punch cards. Way back in the good-old-days when computers were attended by high priests and the card reader was the only portal, chad was a dangerous thing to pick up since it's tiny sharp corners could easily lead to paper cuts. This soft fuzzy edged chad from human punched cards is for wimps.
