Not necessarily. It could be the holes in the voting jigs filled with chad from earlier voters and made it impossible to punch the hole. Or that chad simply stuck to the underside of a card and then migrated where it didn't belong as cards were handled. There are myriad possibilities.

My wife told me she heard a report on NPR about one case where a young lady asked for help when she realized something was wrong and instead an election overseer grabbed her (ruined) ballot, stuffed it in the ballot box, and intimidated her into leaving in tears.
