What is the leverage difference between them?

NDX is the full NDX, MNX is 1/10 of the NDX, and QQQ options are about 1/40 of the NDX.

Also, what about bid/ask spreads and executions? Regular options suck in this regard.

Index options are not much better than equity options in that respect. If you are looking to daytrade the indices I don't see why you would choose options if a liquid futures contract is available. Unless you don't mind leaving tons of profits on the table due to the huge spreads.
