Rather than trying to take money off the table as the stock goes up, I tend to sell PUTs after the stock has gone down. Worst case, I get to own more shares of a company I'd like to own at a lower cost basis than I believed possible at the time I sold the PUTs. Best case I just keep the premium and sell some more PUTs. With ALXN, I've taken well over double my initial investment in shares, so I'm overdue to take an assignment.

Re death endpoint statistically non-significant: Tell Al Waxman that.

Seriously, there are so many overpriced stocks without ALXN's prospects. I fail to understand why someone would take unnecessary risk by shorting ALXN.
