I have seen this happen before. It is usually a prearranged transaction between two institutions that are simply clearing it through the market maker or specialist. There are special rules in this case if you are sitting on the bid or ask because it is not "really" a free market transaction. At least that's what I was told when I called my broker to complain last year.
