Why on earth should U.S. taxpayers be obligated to pay for safe sex in sub-Saharan Africa?

Here's a possible answer. Let's say you get to swap places with someone who has the same relative position in society in Sub-Saharan Africa (SSA) as you have here. Which person would you swap with?

Unless you believe in some sort of manifest destiny of people like yourself, then you have to admit that your position and station are largely the results of the graces that you have granted merely because of an accident of birth.

Let's say you had the same great, loving parents you now have but your family wealth, positions and friends were now peers in SSA. How do you think you'd fare?

My feeling is that we are all on the lifeboat. We have a lot of duty to anyone fully formed and alive now. We also have a duty to those who will come. How could you even formulate the question with a straight face?
