Lately, I'm using the newer sec.gov page:
sec.gov

Leave the top box as is, and in the second one you could enter "4" or leave it blank. I usually leave it blank.

If you enter "4" you get Forms 4 and any other form with a 4 in it like 424A, DEF 14A, S-4.

Information is supposed to be only one day old, using the 5:30 pm Eastern Time cutoff. But it inexplicably is missing the recent Form 4/A and 5.
