is it one account or one account title?

for example: a married couple could have FDIC insured funds up to 500,000 if (with account titles) they did the following:

an account in the wife's name
an account in the husbands's name
a joint account, both names
an account in the wife's name in trust for the husband
an account in the husband's name in trust for the wife

at 100,000 each.
