If you run a fair number of positions its important to keep tract of cost factor data:
Here is a active sample of mine that others might wish to copy or improve upon.

AMKOR (AMKR)

7-11-00.... 100 shares at 3332.00 (includes commission costs)
1-22-01.....100 shares at 2270.00 " "

5602.00 =''s $2801 cost per 100 shares

Calls written:

9-15-00.. sold one Sept 35 call for 1 3/4... 175...-27 (com.).net +148
10-31-00 sold one Nov. 22 1/2 call for 1 1/2 150-27.............net +123
11-22-00 sold one Dec22 1/2 call for 1 1/4 125-27..... ..... net + 98
02-12-01 sold two Mar 20 call for 13/8 = 275-27................. net +248
03-22-01 sold two April 20 calls for 1 1/4=250-27 .............. net +223

Subtotal of call premium is +$840.00

cost of stock was $5602 minus $840.00...='s 4762 or 2381 per 100 shares.

This is an example of a minor option play. In years past I would have sold
the stock and moved on to something else. If some one has a better worksheet
post it up here so we can all consider using it.
