Well, it appeared to me that the rack-mounted servers were dual fanless-palomino, and a fanless chip is likely to operate at a lower voltage, right?

were do you fix a fan on that kind of heat sink?

The AMD760MP server demonstration system from VA Linux used only passive cooling (big heatsinks) for the two CPUs and one fan for the entire case. The laptop techdemo (1 GHz) had active cooling but the heatsink was cold when you touched it. I'm not sure about the real notebook system but probably it had some sort of temperature regulated fan.
