I thought the main problem with low Vt devices was leakage current. Are there problems with switching current too?

Well, both higher drive currents and much higher off currents lead to considerable higher heat generation in the localized areas where these devices are used. Thus, they must be chosen judiciously and used in limited areas where needed. But, in looking back at what Intel has published, it does not appear that they did use low Vt devices in the .18um generation process. They do indicate them for the .13um generation. So, what I said is not likely the problem.
