It is most usual to relieve symptoms of shortness of breath within 1 hour and see improvement in arterial blood gases within the same time frame. There are groups of patients who are in extremes many of which will require urgent intubation and mechanical ventilation. These patients may require longer periods to improve. Frequently this group may be hypotensive requiring addtional therapeutic measures. I just don't see a need for an expensive new drug that may not be that more effective than current cheaper alternatives. Will it be used..yes. Will it become the standard of care...I doubt it. In addition since current treatment relies upon multiple drugs, testing this drug only against IV NTG does not tell the whole story.
