Mark Twain once wrote that if a cat sits on a hot stove-top, it'll never sit on another stove-top again -- even a cold one.

I once encountered a completely destroyed system. The user proudly presented me with the back-up tape, and said it was current, having done his duty with it the previous evening.

As you probably guessed, the tape contained one day of data files --- No system, no programs, no history. He wasn't entering the correct commands and he'd been creating one-day backups for years. This was a case of human error in two parts: One, his back-up procedure was flawed; and two, his back-ups were never put to the test.
