I believe it means -
The drilling mud carrys away some of the material on the sidewalls of the well, which results in a gap and repalcement by mud related material. Happens with stuff that isn't well stuck together, like sandstone with big pore spaces.

When you have a big enough opening around the well bore, the arms of the loging tool (sonde) can't reach out far enough to make contact, so you don't get log info from that area.
