Current VOD delivery systems are being developed based on the cable TV industry model, not pure VOD over the internet. Content is downloaded onto the cable systems and in most cases is stored at the 'head end' where the 'last mile' pipes are multiplexed. So if you were to get hundreds of thousands of requests for the same video at the same time, it's possible that each cable head end would need to download the content once and then multiple users within the cable system would now have VOD control over the content and the bandwidth issue would shift to the headend/last mile from the core. Similar situations could be developed by NSPs/ISPs to dramatically reduce core internet traffic by storing all 'multiple' video requests in a cache and eventually removing them from 'instant' access once 'demand' has fallen off.
