Do you mean in the general population, or just in prisons? The big problem, as I see it, with the government doing clean needle exchanges is that, technically, they are aiding and abetting the commission of a crime. That's not just a legal issue, but a moral and political one. If we say "hey, people are going to commit crimes, rather than focus all our efforts on stopping the crimes let's put some resources into making sure they commit their crimes safely" that opens up a huge can of worms I'm not sure the government wants to open.
