No, it wouldn't change the former, but it could make people with an unofficial family but no formal family eligible for transplants. It seems reasonable to me to give preference in transplants to those with the best chance of recovery and there's apparently a correlation between chance of recovery and a strong support system, which is currently defined as official family. Having legal relatives isn't necessarily a good measure for having a strong support system, but that's what they use. My system would fix that inequity, among other things.
