the Dog's Bollocks
colloq. - the absolute best.

I don't know the pub but there's a reasonable beer by that name... with a label that got censored.
I shudder to think of the derivation, however.

Second-best use of the word I've heard was in an army drill chant...
so, in best sar'nt gruff:
"yer bollocks are swingin' from left ter right and yer left - right - left..."
Failing that clue, change dictionary <vbg>

how did he achieve it such that he is now walking around loose?

He was, in his raucous youth, a notorious football hooligan, one of the lager louts that travels the continent starting riots at every opportunity. This habit periodically becomes a political and diplomatic issue; his stint as most wanted occurred at a time when the Brits were trying to convince the rest of Europe that they were cracking down.

I can't say I approve much of hooliganism or football riots, but this particular ex-hooligan is a good fellow. In this part of the world you frequently run up against people with all sorts of bizarre backgrounds; you learn to be less judgmental.
