That's where next generation components come in. Increase bandwidth through the same pipes. Historically, the magic financial formula has always been four times the network capacity for 2.5 times the cost of the existing network. If it fits the formula; Bingo, upgrade.

The cost of building new networks is far cheaper than a few years ago. Tier-1 players that let bandwidth prices rise in the face of growing demand, would be begging for increased competition from more efficient players...not a good idea, IMO. They will scale the networks.

Good question, I've been thinking for quite awhile now that SPs have underpriced bandwidth to businesses. But time will tell. There are tremendous cost savings to be had by businesses transacting over the web. Some percentage of that is likely to fall in the hands of the SPs that can figure out how to charge appropriately for it. One of the theories I like best right now is likening bandwidth to electricity. At some point in the future, I bet it becomes a tradeable commodity just like electricity. That's when we'll have great market efficiencies.
