[Warning - pie in the sky, impractical, idea coming]

That's an interesting idea. Rather like the way that standard option Blue Cross has been cleaning high option's clock. It would be interesting to cost out the charges for otherwise "free" services. It might be an interesting exercise just to itemize them and see what's really there. Since telephone deregulation, they've been charging explicitly for all that overhead stuff and I'm startled by how much those little things add up. It would be really cool to see what the Plan A bill would add up to.

My belief is that if this scheme were implemented honestly, eventually everyone would be on plan A and plan B would wither on the vine.

I'm not so sure. Many people believe in looking to the state for solutions when they have problems. You might get a lot of people who want B. You might get others who think they want A but then when they have problems they want to switch to B, and liberals will support them, declaring that anyone who wont let them switch to B is being mean and extremist and doesn't care about children...
