I think Brian is remembering the USS Vincennes, an AEGIS cruiser that splashed an Iranian airliner in the Persian Gulf. The Captain and several officers were held publicly responsible, even though the ship's records showed the airliner had deliberately imitated the profile - flight path and transponder codes - of an (Iranian) F4 Phantom attack aircraft.
