Thou shall not kill is a pretty clear and VERY old suggestion - note, it did not take more than Eve and Adam and their oldest son to start it. But the Cain-Abel kind of homicide is of a man-to-man, face-to-face kind. Homer/Illias like. What we are discussing here, are later additions/versions, that have to do with (...sigh...) civilisation, society etc.

Hi CB,
Has it ever been really unthinkable to attack civilians, or do we just pretend this?

Now you've got it. It's in the genes. We only have civilisation as a survival tool. It helps us maintain our ascendancy over the natural world. Top dog in the food chain.
