In most cases the controllers are paying attention to the their screens and their traffic. In controlled airspace they're not usually looking for (or expecting) an uncontrolled aircraft. Whether the plane came straight in or did a 360, they should have been able to visually see it and they just plain didn't because it's not what they were looking for or at.
