This is a truism of all microbial pathogens: they don't kill all of their hosts. Some have natural mutations that render them resistant. Thus the common cold could wipe out a tribe in the jungle...because they had no natural immunity. I know one guy who is being studied because he has natural resistance to HIV. Nonetheless, anthrax is pretty virulent and engineered in this case to be more so (Iowa Ames strain is especially virulent) and in addition because of the micron size far more easily infecting.

Apparently the Hamilton New Jersey's mail was spiked:

I don't know how they are spiking/targetting regular mail and what their criterion are. Maybe a lot more is spiked than we know but most people still aren't getting it because of the chance that a spore gets into a cut or into the lungs.
