There could be a simple explanation for Mrs. Nguyen - cross-contamination of her mail from the other letters travelling through the mail, and she might have had a compromised immune system. Not just HIV but a number of diseases give you a compromised immune system, and so do a lot of drugs, e.g., cancer chemotherapy. On the other hand, you'd expect either her apartment or her place of work to be contaminated, too, depending on where she handled the hypothetical contaminated letter. Maybe she sat next to the perp on the subway. Maybe she was somehow involved in the plot and was very, very good at covert operations. Maybe the perp tried to infect people on the subway and she's the only one who succumbed.
