I wouldn't say often, although sometimes would be enough to still raise the question so it doesn't have to be often.

There is no way you are going to have a situation in this country where the law requires that a woman die to save her child, even an already born child. It could be considered reasonable to require someone not to kill someone else even if it causes them great dificulty but if that someone else is killing them (even if it is unintentionally) I don't think I would tell them "you have to die".
