If I were a guessing man, I would say they implant Al on the surface and then selectively oxidize (possibly by ion implantation as well) to sapphire to get the "insulating layer", sapphire will be 10,000 better a barrier than silicon dioxide, and if they do it "smartly" and the sapphire layer is no more than 15 angstroms they can handle the mismatch automatically.
