I'm late to this discussion, but perhaps there is one more issue to consider. I apologize in advance if it's been considered already.

You mentioned that you never experienced slippage while trading one or two contracts for nine months, but got nailed when starting to use leverage.

Most security exchanges provide for auto-execution of small lots. Equity options orders under 10 contracts, for example, are frequently auto-executed (but not always).

Is it possible your order was larger than the auto-execution system was set to handle and was delayed while routed elsewhere? In calm markets the delay might be minimal, but in a fast market the delay could have been sufficient to explain the slippage.
